NDIS checklists
Free step-by-step NDIS checklists — applying, preparing for a planning meeting or plan reassessment, choosing a provider, moving into SIL/SDA, and starting with a support worker.
- Applying to the NDIS — access checklist — Everything to check and gather before you ask to become an NDIS participant.
- Preparing for your NDIS planning meeting — What to bring, think about and ask so your first plan reflects the support you really need.
- Preparing for a plan reassessment (review) — A week-by-week countdown so your reassessment is backed by fresh evidence and clear goals.
- Questions to ask before choosing an NDIS provider — The questions that separate a good provider from a poor fit — before you sign anything.
- Moving into a SIL or SDA home — What to check about the home, the housemates, the support and the agreements before you move.
- Starting with a new support worker — Set up a safe, respectful working relationship from the very first shift.
- Switching to a new NDIS provider — Move to a new provider smoothly, without a gap in your support.
- Managing and tracking your NDIS budget — Keep your funding on track so it lasts the whole plan and covers what matters.
- Interviewing and hiring a support worker — Find the right person — safely — when you self-manage or hire support workers directly.
- Leaving school with the NDIS (transition to adult life) — Plan the move from school to work, study or community as an NDIS participant.
What these checklists are
These are free, plain-English checklists written by Novida for the moments that matter most on the NDIS — applying for the first time, preparing for a planning meeting, getting ready for a plan reassessment, choosing a provider, moving into a supported home, and starting with a new support worker. Each one turns a big, sometimes stressful step into a short list of things to check, gather or ask.
How to use them
Open the checklist that matches where you are, work through it at your own pace, and tick items off as you go. They’re general guidance, not personal advice — every plan and situation is different — so use them as a starting point and adapt them to you. A support coordinator or an independent advocate can help you work through any of them.
Who they’re for
Participants, families and carers, nominees and support coordinators all use checklists like these. You don’t need an account, and everything is free. When a checklist points to a form or a letter, you’ll find the official form and a matching template elsewhere in Novida’s Resources.
NDIS checklists — common questions
- Are these NDIS checklists official?
- They’re written by Novida as plain-English guidance, not official NDIA documents. They’re built on the standard NDIS process and point you to the official forms and sources where relevant. Always confirm specifics with the NDIS, your planner or plan manager, as rules and timeframes can change.
- What should I bring to my NDIS planning meeting?
- Bring your access letter and NDIS number, recent reports from your treating professionals, evidence of how your disability affects daily tasks, a list of your current informal supports and providers, and your goals for the next 12 months. Our planning-meeting checklist walks through each of these plus the questions to ask.
- How early should I prepare for a plan reassessment?
- Ideally start about eight weeks out: list your goals and what’s changed, and ask your providers for progress reports and updated quotes. Gather evidence four to six weeks before, and bundle everything one to two weeks before the meeting. Our plan-reassessment checklist gives you a week-by-week countdown.
- What questions should I ask before choosing an NDIS provider?
- Ask whether they’re registered, whether their services match your plan, how soon they can start, whether you’ll have consistent workers, whether prices sit within the Price Guide, their cancellation policy, how they screen workers, and how their complaints process works. Ask for a draft service agreement before you sign.
- Do I need a support coordinator to use these checklists?
- No — they’re designed so you can use them yourself or with family. That said, a support coordinator or a free, independent advocate can help you work through them and prepare, especially for planning meetings, reassessments and provider decisions.
Novida is an independent directory, not the NDIA. We explain each form in plain English and link you to the official copy — always download and submit the current version from the official website, as forms are updated from time to time.
