What the NDIS Consent Form is
The NDIS Consent Form lets you give the NDIA permission to do certain things with your information. It can allow the NDIA to share your details, or let someone you trust, such as a family member, friend, provider or advocate, talk with the NDIA on your behalf.
It's a simple but important tool that puts you in control of who is involved in your NDIS journey and what information can be shared. You decide who, and you decide how much.
What it does
By completing a consent form, you're telling the NDIA that a particular person is allowed to speak with them about your NDIS matters, or that your information can be shared with a nominated person or organisation. This makes everyday tasks easier, like a support person calling to ask a question or follow up on your behalf.
It's worth being clear on one thing: consent is not the same as a nominee. Consent means someone can talk with the NDIA about your situation. A nominee is a bigger step that lets someone make decisions for you. With consent, the decisions still rest with you.
Who needs to use it
Consent is useful for any participant who wants someone else to be able to communicate with the NDIA, or who is happy for their information to be shared with a trusted person or provider. This might be a partner who helps manage appointments, an advocate supporting you, or a provider coordinating your services.
You don't have to give consent to anyone if you don't want to. It's entirely your choice, and you can keep it as broad or as limited as you're comfortable with.
When and how to use it
Use a consent form whenever you'd like someone to help you deal with the NDIA, or when you want to control how your information is shared. You can give, change or withdraw consent at any time, so it flexes as your circumstances change.
You can manage consent by completing the form, by phoning the NDIA, or in writing. If you decide you no longer want someone to have consent, you can withdraw it whenever you like, and the NDIA will update your records.
About the NDIS Consent Form
- What is the difference between consent and a nominee?
- Consent lets someone talk with the NDIA about your NDIS matters or lets your information be shared, but you still make the decisions. A nominee is a bigger step that authorises someone to make decisions or receive information for you. In short, consent is about communication; a nominee is about decision-making.
- How do I let someone talk to the NDIS for me?
- Complete an NDIS Consent Form naming the person you want to speak with the NDIA on your behalf. You can do this by form, by phoning the NDIA, or in writing. This lets them communicate with the NDIA about your matters, while you keep the right to make your own decisions.
- Can I withdraw my NDIS consent?
- Yes. You can give, change or withdraw consent at any time. If you no longer want someone to be able to speak with the NDIA or share your information, you can withdraw consent by form, by phone or in writing, and the NDIA will update your records. The choice is always yours.
- Does giving consent let someone make decisions for me?
- No. Consent only allows someone to communicate with the NDIA or lets your information be shared. It does not give them power to make decisions about your plan or supports. If you want someone to make decisions on your behalf, that requires appointing a nominee, which is a separate and bigger step.
- Can I give consent to a provider or advocate?
- Yes. You can give consent to a range of people, including family, friends, providers and advocates, so they can talk with the NDIA or receive your information. You choose who, and you can make the consent as broad or limited as you like. You can change or withdraw it at any time.
