Setting Goals for Your Reassessment
How to write NDIS reassessment goals that justify the supports you need: showing progress, setting new goals, and linking goals to funding.
Goals are the heart of an NDIS plan. They are the reason your supports exist, and at reassessment they help explain why you need the funding you are asking for. This guide shows how to reflect on the progress you have made, write goals that clearly justify your supports, and set new goals that point to the next stage of your life.

Frequently asked questions
Why do goals matter so much at reassessment?
Because your supports are funded to help you work towards your goals. A clear goal shows why a support is reasonable and necessary, so goals are the thread that connects your daily life to your funding.
How many goals should I have?
A handful of clear, meaningful goals usually works better than a long list. Aim for goals that each connect naturally to a support you are asking for, rather than trying to cover everything.
Can a goal be about maintaining what I have?
Yes. Maintaining capacity is a legitimate goal. Some supports exist to help you keep skills or independence that might otherwise decline, and a maintaining goal makes that purpose clear.
What if I did not fully reach my goals?
That is fine and worth explaining. Reflecting on what worked and what did not helps you set better goals for the next plan. It shows clear thinking rather than failure.
How do goals connect to funding?
Each support in your plan should link to a goal. If a goal points clearly to a support, that support is easier to justify as reasonable and necessary. Vague goals leave supports looking disconnected.
Who can help me write my goals?
Your support coordinator, LAC, or a trusted family member can help you shape your goals, and a therapist's report can help put your needs into words. See Evidence for a Reassessment for how reports support your goals.
