What it means

A plan reassessment is the review of your NDIS plan, where the National Disability Insurance Agency (NDIA) checks whether your funded supports still fit your needs and goals, and updates your funding accordingly. It was previously known as a plan review, and you may still hear both terms used.

A reassessment is a chance to look back at how your current plan worked and to look ahead at what you need next. The NDIA considers whether your goals have changed, whether your supports are helping, and whether anything in your life has shifted. The result is an updated plan for the next period, which may keep your supports the same, adjust them, or set out new funding that better matches where you are now.

In practice

Reassessments can be scheduled to happen as your plan approaches its end date, or you can request one earlier if your circumstances change. An early reassessment is often triggered by a change of situation, such as a change in your health, living arrangements or support needs.

It helps to prepare. Think about which supports worked well and which did not, whether you met your goals, and what you want to focus on next. Gathering recent reports from your therapists or doctors can support any request for changed funding. If you use a support coordinator, they can help you get ready and speak to the outcomes you are seeking. Being clear and specific about your goals and needs makes the conversation more productive and helps ensure your next plan reflects your life today.
