Frequently asked questions

How long should a support coordination report be?

As long as the evidence requires and no longer. An implementation/progress report is commonly two to four pages; a change-of-circumstances report can be one page if the change is discrete. Length is not the measure — a short report that clearly links need, evidence and request beats a long one that buries them.

Do I have to use an official NDIA report template?

No single mandated form exists for these reports. The NDIA publishes some request-specific forms and may ask for particular information, so always check any instructions attached to a request. These templates are adaptable starting points, not official forms.

Can I bill for the time I spend writing reports?

Report writing done as part of coordinating a participant's supports is generally claimable as support coordination against their plan, at the applicable price limit for your level. Record it accurately and claim promptly — the claim window is proposed to shorten to 90 days from 1 December 2026. Confirm current rules in the NDIS Pricing Arrangements and Price Limits.

What consent do I need before sharing a handover report?

The participant's informed consent to share their information with the named recipient, covering what is shared and with whom. Record the date and method. Sharing personal information without consent risks breaching the NDIS Code of Conduct and privacy obligations.

How do I handle a conflict of interest in a report recommendation?

Disclose it in the report, show that the recommendation came from the participant's goals and independent evidence, and demonstrate the participant had genuine choice. If your organisation also delivers the recommended support, that connection must be transparent to the participant and the reader.
